Position Purpose: Streamline and facilitate the decision making on admission to services for medically complex clients. Build a positive working relationship with outside providers along with medical and access staff to ensure client receives seamless services from inquiry through admission. Perform admissions of client throughout the region.

Job Requirements:

  • Licensed Nurse in Illinois
  • Experience in hospital setting managing cases of liver failure, heart disease, and neurological disorders
  • Experience in residential care settings
  • Strong understanding of medical management protocols for withdrawal
  • Ability to understand the capabilities of various Physicians and Nurse Practitioners
  • Excellent communications, interpersonal and leadership skills
  • Computer proficiency required
  • Ability to navigate and complete documentation in the electronic healthcare record
  • Ability to accurately complete required documentation within the prescribed time frames
  • Belief in the mission and vision of Rosecrance

Essential Responsibilities:

  1. Work hours prescribed by leadership and be available to work additional hours when necessary.
  2. Responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse’s obligation to provide optimum client care.
  3. Promotes, advocates for, and strives to protect the health, safety, and rights of the client.
  4. Assume nursing case management responsibilities for designated clients.
  5. Builds and maintains professional relationships with colleagues, interdisciplinary teams, and outside providers.
  6. Be sensitive to and act upon any medical or emotional crisis.
  7. Document to ensure the client service record is maintained and in compliance with all applicable regulations and Rosecrance policy.
  8. Document and follow through with all physician orders and communicate directly with physicians and APNs for approval of admissions after assessment.
  9. Able to demonstrate competent use of all medical equipment and technology.
  10. Obtain and review medical records/information to determine medical acuity, treatment accommodations/needs, and stability for placement into either inpatient or outpatient settings.
  11. Communicate medical requests needed by Rosecrance to safely treat potential clients with either professional referents or clients
  12. Provide Nurse to Nurse staffing to facilitate admissions/transfers from hospitals/rehab settings.
  13. Determine if client meets admission criteria.
  14. When necessary, communicate denial of admittance due to exclusion criteria. Provide alternative resources if appropriate.
  15. Assesses and documents patient status through a virtual interview and performs admission assessment.
  16. Completes required documentation as appropriate, including documenting in the medical record and event reports.
  17. Enter all client medication orders received into EHR.
  18. Demonstrates technical knowledge of equipment and the ability to troubleshoot when problems arise.
  19. Acquires and maintains current knowledge and skills in evidence-based nursing practice and advances in virtual nursing practice.
  20. Demonstrates professional accountability for own practice as evidenced by seeking resources and continued self-development.
  21. If deemed necessary by Nursing Leadership, requirements to provide direct, hands-on patient care or take a patient assignment may occur.
  22. Ensures all aspects of technology is functioning properly prior to usage.
  23. Proficient at conducting virtual assessment and documenting admission client care processes.
  24. Effectively communicate client needs with physicians and APNs.
  25. Communicate thoroughly and in a timely manner with on-site team in relation to client care coordination.
  26. Analyzes clinical situations in a systematic way and acts to obtain resources or correct problems to meet or exceed expected outcomes or patient needs.
  27. Demonstrates ownership and persistence to ensure client needs are met in the best way possible.
  28. Consistently prioritizes work based on changes in patient condition, changes in assignment and unanticipated interruptions.
  29. Support a work environment that is responsible and sensitive to the needs of a diverse staff and clients.
  30. Maintains appropriate therapeutic and professional relationships.
  31. Promotes, advocates for, and strives to protect the health, safety, and rights of the client.
  32. Serve as a member of the Access team and participate in all team meetings and activities.
  33. Understand and comply with the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.
  34. Perform all responsibilities in compliance with the mission, vision, values, and expectations of Rosecrance.
  35. Able to provide culturally competent and exceptional customer service consistently to every customer.
  36. Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
  37. Assume other related duties as assigned by leadership.

Schedule: 8-hour shifts

  • Shift: Monday - Friday 8:30am - 5:00pm (Remote Schedule Available, must reside in IL)

Work Location: Rosecrance Ware Center– Rockford, IL
