(GOBHI) has an immediate opening for a Behavioral Health Navigator. This position will be based out of the employee’s home office, which can be located within the State of Oregon.
The GOBHI Behavioral Health Navigator will work with members and community partners to ensure coordination of care for behavioral health needs. This position works with community mental health providers, primary care staff, and emergency department staff to ensure that members are able to navigate the behavioral health system, schedule timely access departments, help meet quality incentive measures and creates a seamless coordination system.
This position will also assist clinicians within the care management team to relieve any member barriers to accessing behavioral health care as well as help identify various social determinants of health that a member needs assistance with and help connect that member with appropriate resources. This position will also serve as support to the clinical services team at the direction of their supervisor; this may include assisting with care management and utilization management duties as needed.
Essential duties include:
- Strong knowledge of resources in each region of EOCCO.
- Connects members with behavioral health resources by coordinate care between primary care, emergency departments, and behavioral health providers.
- Connects members to other community resources, including housing, employment, nutrition services, etc.
- Monitors progress of care coordination plans through scheduling appointments, documents case records with all information related to the progress of clients.
- Assists GOBHI in tracking member outcomes.
- As directed by supervisor, this position will work with CM/UM teams to maintain workflow productivity; completing UM authorization requests in a timely manner as assigned; answer provider and member calls, transferring when appropriate, and; provide back up to the Clinical Services Coordinator.
- Performs other related duties as assigned.
Requirements
What is needed to succeed in this role:
- Two years’ experience in healthcare administration or case management.
- Strong knowledge of community resources, including behavioral health providers.
- Ability to work well with community partners, finding ways to collaborate through effective and clear communication.
- Excellent time management and organizational skills.
- Ability to communicate effectively at multiple levels.
- Possesses a growth-mindset and is able to find innovative solutions to problems collaboratively.
- Skilled in case management, solid writing skills and ability to prepare documentation and written reports.
- Technology Savvy – proficient in Microsoft Office software and/or Google Suites software.
- Maintains good driving record and required vehicle insurance.
- Education – High School diploma or equivalent.
- Qualified Mental Health Associate (QMHA) certification or ability to obtain.
- Bilingual in Spanish preferred.
