Acentra seeks a Family Support Coordinator to join our growing team.

Job Summary:

The Family Support Coordinator is responsible for providing support and guidance to families who are experiencing difficulties in their daily lives that may lead to residential placement for their child. This position works closely with families to identify their needs and connect them with the appropriate resources and services.

*Position is remote but candidates must reside within the state of Virginia.

Job Responsibilities:

  • Connect families with appropriate community resources and services related to the family's needs and concerns.
  • Assist with the development and implementation of individualized service plans for each family.
  • Provide ongoing support and guidance to families upon service admissions and up to 90 days post-discharge.
  • Collaborate with other professionals and healthcare providers, to ensure that families receive comprehensive support.
  • Maintain accurate and up-to-date records of all interactions with families.
  • Participate in team meetings and training to enhance knowledge and skills.

Requirements

Required Qualifications/Experience:

  • Bachelor's degree in Human Services field and a minimum of 2 years of experience working with families in Behavioral Health or substance use disorders; or an equivalent combination of education and experience.

Preferred Qualifications/Experience:

  • Experience with Medicaid.
  • Lived experience as a parent or guardian of a youth that has received behavioral health services.
  • Strong communication and interpersonal skills.
  • Knowledge of community resources and services.
  • Ability to work independently and as part of a team.
  • Strong organizational and time-management skills.
  • Ability to maintain confidentiality and professionalism at all times.

Compensation

The pay range for this position is $20-23 per hour.
