This Health Coach position provides Kapnick clients with telephonic health coaching services.
The Health Coach's essential functions include:
- Responsible for contacting participants who are program-eligible for enrollment
- Effective Time Management: Conduct coaching sessions promptly at the time of the scheduled appointment and within a time limit of 10-15 minutes for subsequent calls and 20 minutes for the initial call, allowing 5 minutes allocated for notetaking within the specified timeframe.
- If a participant does not answer for their scheduled appt on your first outreach, please attempt 1 more call to connect with the participant.
a. If a participant is a no-show for their scheduled appointment, make two more attempts. In the following week, reach out once more. In the third week, try contacting again and sending a follow-up email. Cease further outreach if there's no response after the third phone call attempt and email. Be sure to document all your attempts where applicable.
- Provide personalized and holistic health coaching services to individuals, including goal setting, lifestyle modification, and behavior change strategies that reduce or eliminate health risks.
- Conduct assessments to identify health risks and develop tailored wellness plans
- Maintain detailed records of client interactions, progress, and wellness plans to ensure continuity of care and personalized support.
- Utilize Kapnick Strive-provided client resources when applicable (Ex. Health hubs, FAQ, RAS Coaching toolbox, etc.)
- Communicate promptly with clients and team members in case of rescheduling needs to ensure smooth coordination.
- Track and resolve client issues and involve appropriate team members and management when warranted
- Keeps current with trends and developments related to essential job competencies and demonstrates continued growth
- Other additional tasks as needed to support the team's goals and client outcomes
- Perform your job in such a manner that other employees will consider it a pleasure and privilege to work with you
- Offer innovation and process improvements where possible to help with firm-wide improvements and efficiencies
- Regular and timely attendance is an essential function of the job
- Perform other duties as assigned
To be considered for the Health Coach position, you should have:
- Bachelor's Degree (4-year college or technical school): Required
- 6+ months of experience
- Skilled in utilizing technology for data management, communication, and client support
- Health Coaching certification: Required
- Other wellness certifications: preferred
- Worksite wellness & health coaching experience: Preferred
