Spero Health is seeking a Remote/Telehealth Recovery Support Specialist (Case Manager) to join our awesome team in Kentucky!

*** This position will work with clinics in Western KY (Central Time Zone) ***

*** Candidates must live in the state of Kentucky to qualify ***

This position is for a Recovery Support Specialist (Case Manager) who supports the mission of “Saving Lives, Instilling Hope and Restoring Relationships” in an integrated team with patients receiving counseling, case management and medically assisted treatment. The Remote/Telehealth Recovery Support Specialist (Case Manager) responsibilities include, but are not limited to the following:

  • Conduct a thorough assessment of the patient’s needs, including arranging for specialized services, as needed
  • Develop and implement the treatment plan in collaboration with the patient, including but not limited to use of community resources, vocational training programs, and other needed services
  • Advocate on behalf of the patient for needed services and benefits. Assist in obtaining reimbursement and benefits including, medical assistance, transportation, educations, etc.
  • Provide education, modeling, and support in the areas of daily living, budgeting, money management, and medication management
  • Engage patient in recovery-oriented, patient centered care in accordance with professional standards of practice
  • Complete required Documentation in an appropriate and timely manner
  • Ensuring compliance with HIPPA and CFR 42 part 2 regulations
  • Collaboration with all members of the multidisciplinary team to enhance patient experience and continuity of care

Requirements

EDUCATION:

  • Bachelor of arts or science degree in a behavioral science or a Master's Degree in Human Services

EXPERIENCE:

  • Minimum of 1 year of full-time employment/internship working in a human-services setting OR Master's Degree in behavioral science, as defined above.
