About Our Opportunity

As a Transplant Nurse Case Manager, you’ll play a pivotal role in coordinating complex patient care specific to transplant patients, ensuring high-quality service and effective communication among specialized healthcare teams. Leveraging your expertise in transplant care, you’ll guide patients and their families through the intricacies of their healthcare journey, collaborating with transplant specialists to design personalized care plans, monitor clinical progress, and advocate for the best possible outcomes.

Things You’ll Do Here:

  • Coordinate and manage healthcare services specifically for transplant patients, ensuring comprehensive care delivery and effective communication among specialized healthcare providers.
  • Assess patients’ healthcare needs and develop personalized care plans based on their unique conditions, transplant procedures, and rehabilitation goals.
  • Advocate for the rights, preferences, and needs of patients, assisting them in navigating the complex healthcare system, including pre- and post-transplant care.
  • Closely monitor patients’ progress, adherence to treatment regimens specific to transplant care, and evaluate the effectiveness of interventions.
  • Maintain accurate documentation of patient assessments, care plans, transplant procedures, and outcomes.
  • Generate detailed reports on patient progress, outcomes, and utilization of healthcare resources.
  • Provide patient education on medication regimens, post-transplant care, rejection prevention, and self-care techniques.
  • Offer emotional support and counseling to patients and their families, addressing their unique concerns and fears.
  • Promote health and wellness by encouraging preventive measures, healthy lifestyle choices, and adherence to post-transplant guidelines.
  • Adhere to URAC Standards, CMSA’s Standards of Practice, state, local, and federal laws, as well as policies and procedures relevant to transplant care.
  • Collaborate with internal teams to optimize health plan coverage for transplant-related healthcare.

Requirements

What You’ll Bring to the Team:

  • 3+ years of direct case management experience, with a focus on transplant patient care. Demonstrated ability to develop, implement, and monitor specialized care plans for transplant patients.
  • RN License with an active and unrestricted license to practice in the state of primary residence – compact license preferred.
  • Experience in a deadline driven environment with a knack for organization and detail.
  • Ability to comprehend the consequences of various problem situations and to refer such problems to the appropriate individual (or supervisor) for decision-making.
  • Excellent communication skills to liaise between patients, families, and healthcare professionals.
  • Patience and resilience, especially when faced with challenging situations.

A plus if you have:

  • Additional state licensures
  • Certifications: CCM, CCTC (Certified Clinical Transplant Coordinator) or willingness to obtain CCM in 18 months of hire.
