What You’ll Be Doing:
Grow Therapy is looking for a Care Coordination Lead to administer our care coordination function. This role will be pivotal in ensuring that care coordination requests are processed efficiently and that clients receive timely, seamless care. You’ll be part of a remote-friendly team, working closely with cross-functional partners in Product, Support, Medical Record, Payor Partnerships, and interacting directly with providers and their clients. You’ll be reporting directly to the Head of Clinical Excellence.
As Grow Therapy expands its services and client base, the Care Coordination Lead will be instrumental in ensuring that care coordination efforts meet client needs in a timely and compliant manner. This role is critical for improving operational efficiency, client satisfaction, and clinical outcomes as we continue to grow.
Your responsibilities will include:
- Leading Grow Therapy’s care coordination function, including developing workflows and processes from scratch.
- Managing the end-to-end process for care coordination requests, ensuring that requests are processed within defined timeframes while maintaining high quality.
- Collaborating closely with Product, Medical Records, Payors, and Support teams to create systems and tools that streamline care coordination.
- Maintaining compliance with HIPAA and other regulatory standards, ensuring that care coordination processes meet the highest levels of privacy and quality.
- Tracking and reporting on key performance indicators (KPIs) to measure the success and efficiency of the care coordination function, with a focus on continuous improvement.
The salary range for this position is $107,000 - $140,000 USD.
You’ll Be a Good Fit If:
- You have 5+ years of experience in care coordination, case management, or operational leadership within the behavioral health field.
- You have a proven track record of building processes or programs from scratch, ideally in a start-up setting.
- You are skilled at working in cross-functional teams, collaborating closely with Product, Medical Records, Support, Payors, and Clinical teams to design efficient workflows.
- You have a strong understanding of HIPAA and other healthcare regulations and can ensure compliance in care coordination processes.
- You are a metrics-driven leader, experienced in tracking KPIs and using data to make informed improvements.
- California residency and CCM or CPHQ credential preferred, but not required
