Cleerly is creating a new standard of care for heart disease through value-based, AI-driven precision diagnostic solutions with the goal of helping prevent heart attacks. Our technology goes beyond traditional measures of heart disease by enabling comprehensive quantification and characterization of atherosclerosis, or plaque buildup, in each of the heart arteries. Cleerly’s solutions are supported by more than a decade of performing some of the world’s largest clinical trials to identify important findings beyond symptoms that increase a person’s risk of heart attacks.

About the Opportunity

This Patient Care Coordination role will provide comprehensive care to clients and patients by developing and evaluating each specific case for patients to receive a Cleerly analysis. You will have direct engagement with doctors and their offices, imaging centers and patients. You will work as a liaison between the entities to include scheduling and collection of payment.

Responsibilities

  • Liaison between patients, imaging centers and providers
  • Scheduling CCTA (coronary CT angiogram) appointments with imaging centers nationwide; ability to navigate imaging centers and providers that Cleerly may not have a prior relationship with
  • Collecting payment from patients using a third party payment platform
  • Create a positive patient experience
  • Data entry and tracking patients journeys
  • Key point of contact for care coordination, answer questions and concerns internally and externally
  • Reconcile monthly invoices with finance team
  • Work with the product team to give feedback on the care coordination process to help build future automation and integrate new developing systems
  • Work closely with the sales team to ensure client satisfaction and exhibits expectations of the patient care coordinator services for sales to present for new client agreements

Requirements

  • 3+ years of relevant experience
  • Experience encouraged for: Sales Force, Google Workspace and Slack
  • Worked in a client-facing role in a physician's office or clinical setting
  • Detail oriented with strong organizational skills
  • Exemplifies amazing client / patient service skills and professionalism
  • Team player with excellent communication and presentation skills
  • Organized and can manage multiple patient journeys at a time
  • Self-motivated and willing to jump into tasks as they arise
  • Takes initiative in their work
  • Quick learner and problem solver that works well under pressure
  • Flexibility and agility to work cross-functionally in a fast-paced environment

Salary: $60,000-$80,000

*Total Target Compensation (TTC): Total Cash Compensation (including base pay, variable pay, commission, bonuses, etc.). We hire employees anywhere within the United States and account for geography when determining base salary.
