Acentra seeks an RN Case Manager-Certified Diabetes Care and Education Specialist (CDCES) (Remote U.S.) to join our growing team.

Job Summary:

  • The Certified Diabetes Care and Education Specialist (CDCES).
  • Assists with case management diabetes program development.
  • Serves as a diabetes care resource to colleagues.
  • Carries a case management caseload.
  • Provides in-home, in-person education, and care coordination in defined areas and telephonic support across the United States.
  • Responsible for assessing, planning, implementing, monitoring, and evaluating options and services to affect an appropriate, individualized plan for the beneficiary.
  • Using independent judgment, the CDCES utilizes clinical knowledge and competence, communication skills, problem-solving, and conflict resolution to effectively ensure optimal client outcomes while considering payor requirements.
  • Coordinates beneficiary needs related to community resources, financial assistance programs, and long-term activity of daily living needs outside the benefit criteria of the medical insurance coverage.
  • Works effectively with all healthcare team members internally and externally.

** The primary COVID-19 vaccination is required as you may work with beneficiaries in person. **

** Work Hours: Monday-Friday, 8:00 AM - 5:00 PM in the Eastern OR Central Time Zones. **

Job Responsibilities:

  • Provides care coordination/case management to beneficiaries; assures a high level of clinical knowledge and performance by the clinical team. Fosters a caring philosophy in all aspects of the case management process.
  • Uses independent judgment and discretion to address and proactively resolve barriers impeding the diagnostic or treatment progress.
  • Develop and implement diabetes educational sessions for staff, beneficiaries, and/or families.
  • Interacts and collaborates with the multidisciplinary care team to ensure beneficiary needs and preferences for health services/information are shared; educates beneficiaries about community resources/options; advocates on behalf of the beneficiary.
  • Establishes a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options to meet an individual’s and family’s comprehensive health needs to promote quality outcomes.
  • Maintain open communication with all appropriate parties while adhering to strict standards for client confidentiality regarding client-related information.
  • Complies with all accreditations, organizational, state, and federal regulations and policies.
  • Prepares documentation, status updates, event notifications and other documentation regarding beneficiaries in accordance with regulatory requirements and company policies and procedures.
  • Pursues ongoing education, certification, and self-development to remain current with case management standards and licensure and certification requirements

Requirements

Required Qualifications/Experience:

  • A current, unrestricted RN License in a state or territory of the United States that allows the health professional to independently conduct an assessment as permitted within the discipline's scope of practice.
  • Associate’s Degree in Nursing.
  • 3+ years of clinical experience, with a minimum of 1-3 years of case management or home care experience.
  • 1+ years of experience as a Certified Diabetes Care and Education Specialist (CDCES).
  • Current Certified Diabetes Care and Education Specialist (CDCES).
  • Ability to multi-task and prioritize with variable and sometimes conflicting deadlines; superior attention to detail and demonstrated ability in decision-making.
  • Demonstrates initiative and judgment in the performance of job responsibilities while maintaining professionalism, flexibility, and dependability under pressure.
  • Strong communication (written/verbal), interpersonal, organizational, time management, and communication skills with a strong focus on customer service, including building and maintaining relationships with internal and external customers and facilitating meetings.
  • Ability to work independently and as part of a team.
  • Ability to research/identify and apply appropriate standards of care.
  • Interest in continuous learning and a commitment to staying informed on industry changes.

Preferred Qualifications/Experience:

  • Bachelor of Science in Nursing (BSN) and/or Case Manager Certification (CCM).

Compensation

The pay range for this position is $83,000.00-85,000.00
