Position Overview

As a Care Coordinator, you will play a crucial role in maintaining contact with Nursing Facilities and General Acute Care Hospitals (GACHs). You will determine each individual’s status, preferred language of communication, and promptly gather all relevant information to efficiently schedule Level II evaluations. Your efforts will ensure that evaluations are conducted accurately and timely, supporting our commitment to providing high-quality care and meeting our patients' needs.

Job Responsibilities:

  • Ensure the responsible delivery of comprehensive services to enrollees, effectively advocating to resolve any issues they may encounter.
  • Actively participate in multi-disciplinary team meetings to collaboratively assess and plan coordination efforts, ensuring the provision of quality care.
  • Provide education and resources to beneficiaries, families, and providers, empowering them with the knowledge needed to make informed decisions about their health.
  • Proactively engage in quality management program activities, contributing directly to the achievement and ongoing maintenance of program accreditations.
  • Assist in achieving contractual obligations, including the cost-effective delivery of services and adherence to budgetary requirements, under the guidance of the Lead, Supervisor, or Manager.
  • Accurately report work hours and expenses incurred during the course of position duties in a timely manner.

Required Qualifications, Skills, Knowledge and Experience

  • Minimum of 1 to 2 years of recent experience in a clinical environment, preferably in field delivery.
  • Experience in the public sector, such as Commercial, Medicare, or Medicaid, is preferred.
  • Strong verbal and listening skills, ensuring courteous and professional customer service with clear telephone communication abilities.
  • Proficient PC skills, including proficiency in electronic mail, intranet, and industry-standard applications.
  • Knowledge and familiarity with public sector health coverage systems, such as Medicare and Medicaid.
  • Understanding of the principles, ethics, and precepts of care management service delivery.
  • Proficiency in computer skills, including care management applications, Internet/Web usage, and Microsoft Office (Word, Excel).
  • Strong prioritization and organizational skills, enabling effective management of multiple tasks and responsibilities.
  • Ability to demonstrate purposeful and outcomes-directed critical thinking in daily operational activities.
  • Capacity to receive verbal and written feedback professionally and implement performance and productivity improvements as needed.
  • Skill in interpreting real-time and historical information to inform decision-making and optimize care coordination efforts.

Compensation

The compensation for this role is $21.00 per hour. Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
