Hybrid Remote Registered Nurse (Bi-Lingual) – New York

Remote Focus provides Chronic Care Management, Remote Patient Monitoring, and Telehealth services to Medicare members with chronic medical conditions. We are currently seeking a Registered Nurse Case Manager to join our team.

The RN plays a vital role in providing remote clinical care for patients with multiple chronic health conditions. The RN is involved in reviewing the documentation completed by the clinicians during the telehealth visits with new and active patients. The RN utilizes the necessary information to create an appropriate and patient-centered care plan. The essential duties and responsibilities include:

• Create a care plan for new patients and determine all barriers and challenges a patient may experience related to the care plan goals. Update care plan goals on an annual basis.
• Establish multiple interventions for case managers to properly address the care gaps of the patients
• Determine the appropriate remote patient monitoring devices, interventions, and measurements to be utilized by the patients
• Provide education by communicating with the patient, family member, and/or caregiver
• Engage with other health care providers on the patient’s behalf to reduce emergency room visits and/ or hospitalizations
• Address escalated patient vitals and measurements and refer necessary escalation to the clinical staff
• Submit progress notes and documentation in an organized, timely, and accurate manner as per policy and procedure
• Provide outreach and engagement of the patient to ensure clinical follow-up services are provided
• Provide crisis intervention when necessary
• Review documentation and assist with education and training of case management staff
• Provide clinical training for case management staff
• Perform other related duties as assigned

REQUIRED QUALIFICATIONS

• Active RN state license in good standing in NY
• Multiple state licenses are strongly preferred
• Bilingual: Spanish/English or Russian/English preferred

SKILLS AND/OR EXPERIENCE REQUIRED

• Experience working in virtual care/telemedicine
• Experience working with the geriatric population and working with homecare agencies
• Experience in providing competent clinical services to patients with chronic health conditions or other disabilities
• Effective interpersonal, communication, and teamwork skills required
