Job Description:

As a Licensed Nurse Practitioner with Your Clinical Pharmacist, you will play a pivotal role in our remote patient monitoring program. Your responsibilities will include:

– Establishing patients in our remote patient monitoring program and guiding them through the process.

– Conducting comprehensive medication reviews for patients.

– Providing monthly check-ins with your assigned set of patients to monitor their progress and answer any questions they may have.

– Offering education and support to patients to promote their health and well-being.

– Collaborating with other healthcare professionals to ensure continuity of care and optimal patient outcomes.

– Maintaining accurate and up-to-date patient records in compliance with company policies and state regulations.

Qualifications:

– Must be a licensed Nurse Practitioner in the state of Oklahoma.

– Experience in remote patient monitoring or telehealth is preferred.

– Strong communication and interpersonal skills.

– Proficiency in electronic health records and other healthcare technologies.

– Ability to work independently and manage your own set of patients effectively.

– Passion for patient care and a commitment to delivering high-quality services.

Compensation is based on the number of appointments rendered. This includes:
