Our expert, Part-Time Nurse Practitioner will serve as the resident expert for medication administration and urgent patient care.
At Brilliant Care, we proactively identify at-risk hypertensive, diabetic and CHF patients and provide them with personalized access to a nurse care manager who works as an extension to the provider. Using high-touch care coordination and advanced remote technologies, we improve patient compliance and medication adherence while reducing unnecessary ER and hospital visits. Essentially, we help improve outcomes substantially, without heavy lift or any out-of-pocket costs for healthcare organizations.
The Position
This position will assist healthcare providers and practices to successfully manage their care management and remote patient monitoring program and provide remote patient monitoring services to patients who are enrolled in the program. Must have a passion for educating patient on how to manage their chronic conditions, be self-driven to meet required monthly goals, and have ability to maintain flexibility in a constantly changing environment.
Duties & Responsibilities
- Overseeing medication management for patients enrolled in the Brilliant Care program
- Conducting thorough patient assessments and making medication adjustments based on Guideline Directed Medical Therapy as needed
- Monitoring for desired outcomes after medication changes are made
- Collaborating with ordering physicians and other healthcare professionals to ensure coordinated
- and integrated care
Supervisory Responsibilities
- No immediate direct reports
- Educating patients and their families on preventive healthcare measures and treatment plans
- Participating in quality improvement initiatives and contributing to the development of clinical protocols
- Maintaining accurate and up-to-date medical records in accordance with regulatory standards
Skills & Qualifications
- Master’s or Doctorate degree in Nursing from an accredited institution
- The ideal candidate MUST have a strong background in guideline-directed medical therapy
- Current valid compact Nurse Practitioner license
- Minimum of 10 years of work experience as a Nurse Practitioner
- Proven experience in guideline-directed medical therapy
- Exceptional organizational and prioritization skills to manage a full patient load
- Excellent clinical and diagnostic skills with a strong focus on evidence-based practice
- Experience working with Diabetic, Congestive Heart Failure and Hypertensive patients
- Exceptional interpersonal and communication skills
- Ability to work effectively in a collaborative, team-based environment
- Strong commitment to providing compassionate and patient-centered care
Work Environment
- Fully remote – must maintain a professional remote working environment
- Residence must be in the state of New York
Compensation
- Position offers an hourly W2 Rate between $60 and $80 per hour
- Compensation is commensurate with level of experience in Clinical Nursing
Preferred Experience
- Doctorate degree is preferred
- Experience in Transitional Care Management, highly preferred
