Key Responsibilities:

• Collaborate closely with our team as an advanced practitioner to help the practice secure contracts for medication management referrals. This strategic role involves no initial face-to-face patient contact but focuses on enhancing our ability to provide comprehensive medication management to patients in need.
• Work diligently to establish and maintain referral relationships for medication management services, ensuring a broad network of care for patients seeking these specific treatments.
• Once contracts and referrals are established, transition to providing direct medication management services, including assessing patients’ mental health needs, prescribing appropriate medications, and monitoring treatment progress.

Qualifications:

• Robust knowledge of psychiatric disorders and psychopharmacology, emphasizing evidence-based treatment modalities.
• Exceptional skills in managing the mental health needs of patients through comprehensive assessment and medication management.
• Proficiency in using electronic medical records systems, with a preference for those experienced with Charm EHR.
• A collaborative spirit, capable of working closely with a diverse healthcare team to develop and implement holistic treatment plans.
• Experience in related fields, such as home care or assisted living settings, is advantageous but not required.
