Remote – RN – Care Manager – NYS MLTC / MAP – Bilingual – New York

Compass Healthcare Consulting & Placement is conducting a search for NYS RN Care Managers for MLTC or Medicare Advantage Plan Case Management. Qualified candidates will have prior MLTC and/or MAP RN Care Management experience including LTSS, Dual Eligible and D-SNP members. Position is Remote, doing telephonic Case Management. Must have NYS RN and previous MLTC/LTSS/MAP RN Care Management experience. Bilingual Bilingual Chinese, Bilingual Spanish and other bilingual language abilities in demand.

RN Care Manager Responsibilities:

Reporting to the Supervisor of care management, this position will provide telephonic case management to our members including medical, social, psychological, physical and spiritual needs by developing, implementing and monitoring the member’s care plan in conjunction with the PCP, caregivers and other team members in order to help meet the member’s needs. managed care nurse, managed care rn, nursing, rn, nurse, care management

Qualifications:

• NYS RN license required
• CCM preferred
• At least 1+ years of RN experience in managed care, MLTC
• Bilingual Chinese (Cantonese/Mandarin), Bilingual Korean, Bilingual Spanish, Bilingual Hindi or Bilingual Punjabi
• Prior experience in an interdisciplinary service delivery environment preferred.Working knowledge of Medicare and Medicaid regulations
